The Houston Area Ryan White Planning Council is conducting a Community Needs Assessment survey about the needs of people living with HIV in our area.
CONFIDENTIALLY…
• Inform us about your needs and the services you use
• Tell us how care and services could be improved, and
• Help us understand how to serve the community better
Eligible survey participants will receive a $10 gift card in appreciation for your help. The online survey can be done on a smart phone, tablet or PC. Each access code can only be used one time so please share this info with others who may qualify.
