Club Team Tryouts Continue for 11U, 12U and 13U this Sunday!
1. Notarized Medical Release Form: Print the attached form, fill out and sign in the presence of a notary, and bring with you. Must be notarized BEFORE arriving (no notary will be at the club). Make sure both parent and player sign the form! All fields must be COMPLETE including insurance information.
2. Birth Certificate: We need a photocopy. Please bring with you.
3. USAV Membership ($45): Complete this online before arriving. You must print off membership card and bring with you. Must be current for the 2017-18 club season. DO NOT change the Club Affiliation until AFTER you’ve signed an Offer and Acceptance Letter. For any questions with USAV membership contact the NTR region office at 469-326-1873.
